Came as shown and works great. I've only had it a month, so can't say much about whether a card print will stick to plastic (like the one I got from my insurance company, which is just an envelope with clear plastic on the side). Like others, I wish it had come in more visible colors. The whole reason I got this is so I can do it so quickly if I get pulled over and have to show insurance and registration. This merges with the glove box making it hard to see. I'll probably look for some fluorescent or reflective tape to attach it to. I don't know if the sizes are standard, but both my Ohio registration card and Grange insurance card fit perfectly.
