I bought the medium size to carry my 12 lb cat. I had to buy a small one. It was very roomy. I had no problem placing it there. Just assemble and put on the head first, then tighten the velcro around the neck, then insert the back end and close the back. The only problem was that he had nothing solid to stand or lie on. So he fidgets and tries to put his feet on a flat surface. I had to keep it under my arm. He liked that his head was clearly visible. He didn't feel trapped. I could pat him on the head. The vet never saw him, but he liked him. I am old and handicapped, and the old ferryman and him were too heavy for me. I was glad that I could deal with it much more easily. Generally a great idea. They just need something downstairs inside or outside so they can put their feet up.
