Bought a couple of these for my elderly mother who had a massive stroke a few months ago and lost the ability to read, write and speak. I recorded "thank you", "I want to get ready for bed", "yes", "I need help in the toilet", etc. to help her communicate with her assistants/caregivers. I put little pictures on each button on the side so she can identify them because she can't read. (Entry from bathroom/toilet photo; getting ready for bed/bed photo) She doesn't use them as often as I'd like, but I try to reinforce the idea every time I'm there to get her to use them to use more often to help others understand what she wants/needs. For me, the recording is loud enough and clear enough, easy to understand. The "speaker" holes are on the bottom, so if you place it on a flat surface (table or dresser) it will really turn the volume down - it should be placed on something "soft" or something elevated so you can hear better. I put a piece of "foam insert" on top and they are perfectly audible.
