There is definitely a learning curve, the movement takes some getting used to and works better on some parts of the body than others. It doesn't vibrate, but consists of two parts that roll together to aim at a point in the middle. I found this to be more beneficial for my lower back than my neck as the pressure from the device is very strong and the area is less sensitive for me. However, it is better than a regular heating pad for back pain! It helped me a lot during my menstruation. Be careful not to use it all day as any deep tissue massage will be sore! It turns itself off automatically after a few minutes, I wouldn't use it for more than half an hour or an hour for this purpose. Remember to rest your body and stay hydrated. There are three settings available to adjust the speed of the massage, a button to turn the heating element on and off, and one to switch directions (although this seems a bit pointless given the directions switch automatically every minute or so). The device itself is also a bit bulky, but since it needs to be plugged in, you'll still be motionless. Overall pleased with the quality for the price, looks like something you would order from a quality gift catalogue.
