I bought 40W. I wear a size 7 outdoor shoe (adult female) and it came pretty close, but when the upper on my left foot, which is slightly smaller, broke inwards, I did NOT increase the size as some users suggested. I'm not sure I should have done that. I used the included silicone toe pad and the two shredded cotton balls in the box and it exceeded all expectations. I switched to NY CHQ wool. This comes from sheep that have been entirely hand-raised, home-raised, raised organically, hand-sheared (sheared?), hand-washed and hand-combed. I also use a finger rest and sometimes a foam sleeve if I'm going to be dancing for a long time. The straps are pre-sewn and in the correct position. Later I sewed on rubber bands and Bloch elastics. I will buy another pair if these are no longer available. They break down very quickly. By the way, I can't wear Bloch or Russian Pointe because they hurt my toe. Now I wear Suffolk Spotlight. Because the wings on them weren't high enough to properly support my leg. The color is more of a peachy color like the Suffolk Spotlight and not the pale pink Gaynor.
