The water pulsation is very fast, not just a big stream. It may be because I'm not used to it yet, but it hurts a lot! I have a taste of blood in my mouth after using it. I will continue to use it for a week or two and see if it gets better because my dentist recommended I get this tool. I almost sent it back because it didn't work initially. I had to suck on a straw to get the water out. The one I have is battery operated so you can take it with you on a trip, but it's much larger than I expected and uncomfortable to hold. I suggest using it in the shower as water gets in everywhere! Edit: After using it for a while, I found that it no longer hurt, and the key was to use warm water instead of cold.
