This adapter is useful for separating a standard RJ11 fax machine jack from a multi-line office phone with RJ45 jacks. The phone has a line-in and an auxiliary line connection for the OnQ adapter. Telephone and fax are tied to the same line in telephone or fax mode. Only one of the ports on ONQ worked for me. You can select the custom line you want on the phone by lifting the handset as usual, selecting the line you want (by receiving a dial tone), and then pressing the fax send button on the machine. The input cable of the ONQ adapter is rather short. You may need a Cat5e extension or a cable + coupler. All in all, it's definitely worth the effort.
