IMO potential overheating hazard

The appearance of the product is quite good and it looks very "upscale". The size is good. The price was great so I bought 2. Cons: 1) Poor cable management. The small plastic clip is more difficult to work with than the old Dell rubber strap. Please note that the picture only shows the small cord nicely rolled up. Where does the thicker cable go? Good question. 2) The real problem I'm having is that it goes into thermal runaway mode. Sounds bad and it is. My Dell E6430 has no problem charging when the laptop is off. Moderately warm in the center of the charger. But when I'm working on a laptop with a dead battery, the device heats up so much that it's difficult to keep it outdoors. I first noticed this issue on first use when the charger was charged. the edge of the sofa I was sitting on and my hand touched it. The charger has become so hot that the label on the front has blistered. I absolutely couldn't keep it (no exaggeration). I'm pretty sure it would be out of service within minutes. The big question is how could this error have occurred and would a fire have occurred? I suspect this is a design issue, not a faulty device.
