The patella strap easily adjusts to fit different sizes and stays in place throughout my workout. I wear it to the gym for cycling and it never moves, gets tied down or gets in the way. The gel pad puts good pressure on the front of the knee. I have a torn meniscus due to osteoarthritis and it seems to support all of the painful areas of my knee. I also wear it for long periods of time without further adjustments on outdoor walks. So far I have nothing but praise for the patella brace. I bought the medium one and the strap goes all the way around my knee but fits and is perfectly comfortable. I could wear small but I measured medium.
