Poor manufacturing quality control
The tie was good but the manufacturer didn't take care of it. The tie was wrinkled, but obviously not due to delivery problems. The outer plastic was completely intact, so definitely from the factory. Something like this should be wrinkle free by the manufacturer, who should take reasonable steps to ensure it stays that way.
