Bad design - cable management
The part that goes against the zip tie is too thick (it needs a more acute angle), when you pull hard, the thickness of the tool pulls the zip tie on the attached part, so that it won't. Since I work very tightly, I would look for a tool with a "sharper" angle on the end to provide a better grip on the tie.
