Make sure the book asks for the people's address

It's poorly done, it's already torn from the book. It lets each person take up a whole page and asks people to say something to a dead person. Most people didn't. just wrote the name there all over the big page. Also, he didn't ask for an address. So I had to see them all. Simply made complex.
