Hole drilled too far towards receiver hole. I don't want to start a new article, but if you were in need you could. The protrusions on the end prevent it from penetrating far enough to line up. Also, I had three different 2" constructions, all of which dangled fairly loosely in the adapter just sitting on the bench. A bit normal, but it seems enough that if you don't tug on something, you'll hear a knock when you turn it.
