USB cable plug not seated properly in adapter socket. It feels free and knocks out easily. I bent my toe on the first to improve its fit and had to buy a replacement. Too bad that there is currently the only completely black one. If you don't care, try another brand. *UPDATE* This adapter works well with some E-Type plugs, but not all. Specifically, there is a physical incompatibility between this E-type jack and the E-type plug on the front of the Define 7's body.
