I need a medium or even wide toe due to bad big toe condition but a narrow heel is an impossible combination except in hiking/running shoes. I can even wear wide ones in them because the collar keeps the heel in place. These shoes, which I bought from a medium, were just wide enough in the toe, but I got out of them as soon as I put them on. Also, the toe box is low (i.e. not deep) and doesn't pick up on the ankles. Finally, before buying, I forgot to check: the insole is not removable, so orthopedic insoles do not fit. Generally impassable. This could have worked for me 25 years ago, before my feet rebelled from years of wearing fashion shoes (which contribute to bunions and hammer toes). The leather is soft and for someone with less complex foot issues these are cute shoes to run errands or wear to a part-time workplace. They are definitely way, way cooler than athletic shoes.
