My 3 year old was always in the 40th to 50th percentile range most of the time. We have 3T (he wears great 3T shirts including Old Navy, Carter, OshKosh, Disney, Polo) and the shirt was really cute but it's really big. The neck hole was also very large. We hesitated whether to return or not but it was too close to his birthday party to return. So we rolled up the sleeves and couldn't do anything with the neckline, but it really improved the look of the shirt on him. We'd probably be better off with 2T.
