Definitely not for 4 year olds or 16 inch waist as advertised. Hole for another 4-6 inches. I don't think a girl in my size 11/12 would fit without me cutting another hole as it's not much wider than the little sister. Maybe teenagers and teens who are taller than average and wear adult sizes, but other than that I wouldn't classify it as a "boy" or "kids belt". The smallest waist it can fit without cutting its own hole is 28.5 inches!
