Mechanical error. See figure Step 4, "Attaching the speakers” in the manual. However, the plastic is not strong enough. The screw goes through and renders the mount unusable. I was careful not to overtighten. I wanted to keep tightening until the speaker stopped slipping off my elbow. I noticed that the screw broke through the plastic. Over tightening means continuous tightening after 2 pieces are already fastened. Since they were never safe, I don't think overtightening was ever possible. According to the manual, my speaker weighed less than 8 pounds. This is an awful product. I will never buy anything from Dynex again.
