Breaks into pieces suddenly at 400°F

A large sheet of Pyrex broke into small pieces after 30 minutes in the oven at 400°F. We used it a couple of times but the third time it suddenly banged and all the food went into the oven. Now I have to use another flat plate on the rack below if I use a small pyrex plate just in case.

Pros
