I loved this item in the 1st trimester and started weaning it by the start of the 2nd trimester. However, at the time my stomach was growing fast and I couldn't climb as well at night (thus having to take breaks to urinate up to 5 times a night). Every woman will grow at a different rate, so take my review with a grain of salt. I can't use it at all in the 3rd trimester, it's uncomfortable and I almost feel worse. I also have neck issues (present before pregnancy) and have found that the top of the pillow you put your head on is sometimes unwieldy due to its shape and trying to get it right. It gave me GREAT sleep for the first few months and I would recommend it for that time but not later. I'm 35 weeks now and should instead sleep with pillows around me that are easy to take off and put back on after nights in the bathroom.
