Lost connection and now doesn't appear in Bluetooth list and not via USB

I work in IT and consider myself quite tech savvy. I really liked it when I first got it, but one day it lost its BT connection and it's impossible to set it up again. It does not appear in the Bluetooth list and does not connect via a USB cable. Worked great for 2 months and then "fired".
