Noisy! Heatsink collision on rPi4

I tried this product to run an rPi for a pihole that tends to generate heat. The fan was the loudest in the entire 6U rack in my home office, which also houses a Synology NAS, a 16-port POE switch, and a few other devices. I decided to order another one, hoping the noise issue was an accident. It was not. I bought two versions of this POE hat. The one that came with a case and one that didn't. I really like this case, but this POE hat is too loud for practical use outside of a data center environment. I ordered smaller POE hats with no 12v output and now use cases with fans that I run at 3.3 volts. They are almost silent and my CPU temperature never exceeds 45 degrees Celsius. This thing managed to cool my CPU, but it was too loud. In my opinion, noise implies friction. Friction means wear. Wear almost always leads to failure. For these reasons I do NOT recommend this product and am returning the ones I have purchased, although I love the plain clear plexiglass cases that come with them.
