Fixed my problem in Spares
All fan speeds are back. When I removed the old one, it obviously burned out and dry leaves fell out of the hole. I removed the fan motor and found more leaves inside, so I vacuumed the case with a shop vacuum before putting everything back together. There is no more noise or vibration from the fan. (A YouTube video shows how to do this.)
