I'm 5'11/275. I wear Wal-Mart's sleeveless starter compression shirts to help 'balance' things when wearing shirts etc. Wal-Mart has stopped selling/started changing them and they are shrinking a lot less. moderate resemblance noted. most are either too long or not compressed enough. I bought the MEDIUM size. It's a little tight to put on, but it also stretches a little. I am very happy with it. I'm not uncomfortable at all, but if you're new to wearing compression garments, you can go up a size. I usually wear a "1XL" shirt from places like DXL or 2X in most standard shirts, usually 17.5/18. I hope this helps you determine the size. The only reason I took off a star is because they are "hand wash only" - I still machine wash them but I have a feeling they won't last as long.
