This shiatsu massaging pillow was great right out of the box. I was surprised how revolutionary it was for what I purchased, compared to massaging pillows or seat backs I have tried before. Instead of being just a lame, vibrating massage, this is a true shiatsu device-- similar to what you feel on those super expensive shiatsu recliners (like the ones we all experience at Brookstone or Sharper Image, or found at airports or malls). It is quite strong motion, and you can select heat or no heat with attached switch. I moved it to different parts of my back, and after doing so, felt super relaxed an loose. I wouldn't recommend necessarily putting on a sore part of your back, since it is quite strong, but I suppose you can weaken effect by not pushing as much back onto the pillow. This pillow will definitely "get the knots out" of your back. On your neck, it is also soothing. A good buy, and would also be a great gift to anyone, especially at this price (of around $35, currently). Has a flipping faux leather cover that (removable from attached velcro connection) that can be used I suppose to ease slightly reduce the strength of the shiatsu (or if you want someone to try it and then easily wipe of the smooth surface if you want), or can just keep using with the nice mesh covering. Oh, and it comes with a nice long electrical cord, and even a separate, long cord for your car!
