My cat had an irritation in his eye and was pawing at it constantly, so I got him this until I could take him to our vet. It works well and has plenty of room for adjustments to make sure it fits. The sizes seem pretty straightforward and the medium works well for him (and I suspect most other medium to large housecats). All of that said, my cat did not enjoy having this thing on and tried to paw it off a lot. Fortunately, it doesn't seem likely that he would ever succeed in pulling it off. Interestingly, he didn't mind having it on backwards (with the cone more around his chest instead of his head), and I think that helped him get more comfortable with it at first. Maybe something to try if yours is also unhappy with their new attire.
