Definitely runs large, and they are wider than what I normally consider to be wide. Maybe they are extra wide. My normal size was 42, and they were way too big. I went down to 41 and my toe is still almost 1/2" from the end. They still feel big. I'm afraid to go down to 40 because that's what I wear in rock-climbing shoes and I don't think I need such a tight fit for cycling, do I? The width is probably too roomy for me, and I always go with wide shoes. Only after riding 50 miles do my feet swell into a decent fit. But when first starting out, I had to crank down the two straps up front. The ratchet fits well for me and works well. Don't get me wrong, they are comfortable, and a good solid sole for pedaling on. But the sizing is not as expected.
