My Accord 03 idled poorly (RPM up and down), I bought an idle air control valve from carparts.com ($26), installed it ($50) and cleaned the throttle body. the problem didn't go away. I saw on Amazon that the whole throttle body assembly was cheap ($63), I decided to give it a try and after installing it ($100) the problem went away. It's been 2 months and it's still working perfectly. Now the stock throttle body is $800 which is probably more than my car is worth. I don't expect it to last 5 years but maybe in a year so I can save money and buy a better car. My 03 has 220,000 miles on it, so that tells you the car isn't worth the $800 investment. If you have a newer car that you intend to drive for more than 10 years, I'm not sure I would put a non-original part in it. by the way, there was a hole that was poorly drilled for the throttle cable to fit snugly, so my mechanic had to shave the head of the cable a bit. otherwise everything was good.
