Definitely not for 4 year olds or 16 inch waist as advertised. Not only did I have to trim about 6-8 inches for my size 6/7 girl, but I also had to cut a new hole another 4-6 inches. I don't think a girl in my size 11/12 would have fit if I hadn't drilled another hole as she is not much wider than her little sister. Maybe teens and teens taller than average wearing adult sizes. , but other than that I wouldn't classify it as a "boy's" or "kid's" belt. The smallest waist it can fit without cutting its own hole is 28.5 inches! I give it 3 stars
