Replacement headlight GM 3500HD.

Update: Three LEDs are off. Used to replace the original position light on my 2008 Silverado 3500HD dual. The mounting holes lined up almost perfectly and once tightened it fit well. Much better than the $60 GM part with three low power LEDs.
