These shoes are very snug, I had to order half a size up from my usual size and they are still quite comfortable. The larger sizes still seemed like a bad idea as I use them for court sports and need more lateral stability. Remember that all of the heel pad comes from the insoles! and no midsole. The insole is very thick and doesn't look like traditional insoles at all. That means you'll have a hard time replacing the insole with better arch support or a custom orthotic. Putting something on top of what's there means your heel is too high and your hamstring is slipping out from behind. But the replacement doesn't take up enough space in the shoe, you remove all the cushioning from the sole and the shoe no longer fits. I've had some success with art and making a recessed heel counter pocket for my custom footbed orthotics. It was a gamble, but it seems to be working.
