Long lasting and effective!
I have used this for my neck/shoulders, ribs, lower back, and breasts after breast reduction, all while reclineing. It gets nicely cold and lasts for a while unless it's over 80*F in the room. I have used it for a couple years now and it's still in great shape. I have not used the strap, so I have no feedback for that. Pad does not have a strong smell.
