Apparently 3/4 inch slides are not common. I was able to find a direct replacement for a broken drawer slide here, 3 screws and it's done. I noticed that there were 2 broken rails on the bottom of the cabinet (someone else replaced them beforehand) and noticed that the original installation used tapered wood screws which probably contributed to cracking and ruining the screw holes. I used flat sheet metal uses cap screws (with flat faces) to attach the plastic part to the wooden box - this should prevent the screw holes from cracking. some finesse for positioning.
