My old ones are totally rotted inside and parts have fallen out. They fitted like a charm, but when the water was turned on the pressure dropped by about half. It took longer to rinse out the toothbrush and do something else. My mom even said she thought it was clogged and needed replacing. I took it apart and found that the water should flow through the strainer, then through this small hole to conserve water, then through an element that disperses the water, and then through another strainer. All I did was drill that little pin hole a little more and everything was secured. Be careful not to drill too hard, otherwise there will be no aeration and you will use a lot of water.
