I ordered a small one because my cat is long and skinny. However; Unless you have a very small cat (perhaps a cat that is still growing) I would suggest a medium. The little one is really tiny and the clasps won't fit most underarm cats. This part of the harness cannot be adjusted in length. The waist part can be largely adjusted. Of course, choose large if you have a cat or a large frame collar. I can't see the actual product because I can't use it. But I'll update my review once my medium suspension arrives.
