Absolutely silent. We figured our Russian midget Bobby didn't use it because we'd never heard him walk. Then we noticed dirt building up in the wheel and realized how quiet it is. It's great because Bobby's trash can is a few feet from my light sleeper daughter's bed. The wheel attaches to the stand with a bolt and wing nut, making it easy to attach to the bin - just drill a hole. Possibly add shims if you need to move it off the side. The reason I removed the star is because it's not easy to clean. The cork covering the tread can be wet and cleaned (gently), but the wood in the middle will inevitably get wet too and I'm afraid it will warp. Overall it's a great handlebar, but if I ever have to buy another I'll look for an all-acrylic option, with the exception of the cork tread.
