Looks good but doesn't last after washing

This is my second one and the same thing happened after washing. Does it get wet and mold/mildew stains appear? Nothing comes out. If it never had to be washed that would be great, but babies pee out of diapers or spit up, they need to be washed. The inner pillowcase also fell apart in the wash. The return period has of course expired.
