We have replaced 3 toilets and installed a new tile floor. The base or plastic floor tube that holds the screws is perfect for eliminating "wobble" in your toilet. NO, you do not need hard plastic spacers. You need both some padding for cushioning and the ability to fully blend in and get the best coverage on the pad. Gasket, or even possibly your toilet bowl, depending on where the gasket needs to be placed. You just push them in (maybe someone will push the toilet away from the gasket so you get maximum alignment) or hammer them in with a small rubber mallet or wooden handle and then cut flush or slightly less than flush to hide them. They worked great on all three toilets.
