The instructions say to heat them for 30-45 minutes but after 20 minutes they got too hot to touch. It heats up instantly. I used to always use hot stones with hot water and I was tired of burning my hands in the water so I gave this one a try. I'm very happy with it in a great way. I had to put it on a wooden base as the underside gets very hot. There is no heat regulation. Turn it on, watch it to see how long it takes to reach your desired temperature, and then you'll know how long it takes each time. The stones are very smooth and easy to clean. If the stones seem hot just keep them moving by massaging them and when you place them on the body you can leave a layer between the stones and the body and add another layer of layers if needed. They penetrate deep into the muscle through the heat and help the muscle to relax a little before the massage. When I use the handle on the stone box, I can't hear the stones moving. This is a great heating box and stones.
