I bought the medium one for my hefty 18 lb cat. Fits well, I think it would be possible to take a smaller size. I gave it one star less because the buckle and all is a bit awkward for a cat, but ultimately this is a harness made primarily for dogs. I like the support it gives, I think it's better than those thinner straps for him because they put less stress on his joints and especially his shoulders. He seems to walk with me more often than he wants, like he did with his previous Come With Me Kitty harness. I could tell it threw him off balance for the first day or so, but he quickly got used to it. The front of his neck was a bit too big for him which allowed him to come out once but I attached the velcro more diagonally, tighter near his head and now it fits more securely. I'd say downsize if like me you're an inch in the next size up. The buckle itself is not adjustable and it would be more convenient and secure if there was one (if the straps are a bit large). This harness won't stop your neighbors from making fun of you for walking your cat. The item was shipped quickly.
