Caused by TV high pitched sound
After hours of troubleshooting, changing HDMI cables, inputs, TV settings, input settings etc. I found that this high pitched sound was coming from the TV when it was playing brightly. HDR content and in particular the playback of HDR content via an HDMI line with CEC support. I suspect that the increased power required for CEC and especially HDR highlights is beyond the capabilities of this power cord. The high pitched tone I found is due to either voltage or power supply issues. When I went back to the original cable all my problems were gone.
