I don't find this mat too painful. I couldn't feel it at all through a thin t-shirt, so I applied it directly to my skin while lying on the floor. For a softer effect, you can use it lying on the bed. I have found that a mat and especially a pillow help with tension headaches. I usually use it for 10-15 minutes at a time, and on a bad day you can do it as many times as you like. I'm not saying it's a cure, but it's part of a system I'm using that has relieved me of the severe tension headaches I've been experiencing daily for months. I couldn't go to work for several days. Deep therapeutic massage if necessary. This combo works better than anything the doctors have tried, any pill, and I've gone through MRIs, CT scans, X-rays, chiropractic, etc. with no help.
