I ordered a pack of 20 keystone bushings and had the same problem as others over 18 months ago, although the seller said he found the problem in R&D and development and worked on it. The internal width of the connector is so large that they cannot be stacked side by side (possible vertically, but not horizontally). I was hesitant to buy keystones and wall plates based on the many comments on the subject in general but like many others I wanted to set up a network. I ended up buying Keystones (model CL0080) from CableCreations. I also had issues with a purchased perforation tool (as others have mentioned) damaging a couple of keystones.
