Product fails completely on first use

The reflective coating on the tape is peeling away from the adhesive portion of the tape, preventing the reflective material from adhering to the surface it is attached to. The top piece is a ready-to-use band (which should happen). The bottom part is the separation of the reflective material from the tape, and what's left is some reflective material that doesn't stick to anything.
