The two fuse holders were made from 18 gauge wire rather than 16 gauge wire as advertised. One of the two blade holders had the blade slots offset about 1/8 inch vertically - one protruded above the bottom of the fuse holder. This prevented the fuse from sitting flush with the bottom of the holder, forcing it to sit higher in the holder and preventing the waterproof cap from landing as intended.
