Product fails completely on first use

The reflective coating of the tape separates from the adhesive portion of the tape, preventing the reflective material from adhering to a surface to which it is attached. The top piece is a ready-to-use band (which should happen). The bottom part is the separation of the reflective material from the tape, and what's left is some reflective material that doesn't stick to anything.
