O-ring missing - brew

I put it in the pin on the carbonator lid, turned on the gas and the beer spurted everywhere. I took it apart and found that the black o-ring that seals the pan head screw cap was completely missing. The union nut on the gas hose was also loosened. I added an o-ring, tightened the nut and all is good now. I don't normally give 1 star ratings for an easily fixable manufacturing quality issue, but in this case we are talking about a component of a compressed gas system, so failures like this are safety issues.
