I ordered a medium size as my cat was just on the borderline of medium to large. It's a bit tight but I think it should be so that they can't get out. Anyway I think it works. It's soft, with velcro and a buckle, but when I put it on my cat for the first time it just rolled onto its side and didn't budge. He tried to get up when I tempted him with his favorite dry and wet treats, but he just fell back. I'm embarrassed to laugh at him, but it was pretty funny. I'll try to see if he gets used to it.
