Bladder issues and backpack straps

The first time I used it my bladder leaked under the screw on top and my whole Gatorade spilled onto my couch. At first I thought I screwed it on incorrectly, but after refilling I saw that the leak was coming from under the sealed area where the cap was screwed on. There is no way to fix this. Very disappointing as I was planning to use it on a hot summer day in June in Texas. As for the backpack, it seems to be well designed. The storage was fine for a short hike, but I quickly found that the straps that go around the waist were poorly designed. If I tightened the straps, they would immediately loosen completely. No matter what I tried, I couldn't tighten them. Aside from trying to fasten the straps myself with new buckles that I would have to order, I didn't even bother. I ended up putting a camelback bladder in my pack, which was fine, but as the waist straps were useless I skipped it altogether. Surprised because the reviews were so high. I think I just caught a bad one out of a bunch.
