Broke in less than 3 weeks

I really liked the chair when I bought it for the first time. It was comfortable and just the right size for my standing desk. Unfortunately it broke less than 3 weeks after assembly and use. Now when you sit on it, it falls forward. This happened a day after the return window so I'm stuck with it now.
